ATTENTION

Please read and observe these notes before completing the questionnaire, as it is important that you understand and appreciate these points as the Public Defender undertakes to investigate your complaint —

1. The making of this complaint to the Public Defender does not in any way prevent you from instituting legal proceedings in court. Neither is the making of this complaint prejudicial to you in any such proceedings.
2. The making of this complaint does NOT create a lawyer-client relationship between you and the Public Defender; accordingly the Public Defender does NOT and CANNOT operate as your legal representative.
3. You are at liberty to seek legal advice on any feed-back, suggestion, or report generated by the Public Defender in respect of your complaint.
4. Under the law, the Public Defender is empowered to discontinue at any stage, any investigation or enquiry which has commenced as a result of your complaint, and thereupon close your file. Please note that the Public Defender does not require the permission of any complainant to close that complainant’s file.
5. The cessation of investigation and/or the closing of your file by the Public Defender is not a bar to you seeking redress elsewhere.
6. The receipt and recording of your complaint does not thereby vest in you, any property in the file created by the Public Defender. The file remains exclusively the property of the Public Defender, save and except the documents which were physically produced by you, or by third parties, authorised by you so to do.
